Headphones & amp question

I recently purchased a used Vox DA5. Everything works perfectly except the headphones only have sound in the left ear. I tried a pair of earbuds with the same response. Is this an issue with the amp, the headphones I'm using or something I'm doing?
 
Make sure the mini to ¼" adapter is a stereo mini to ¼" mono.
The line/phone output on the amp should be a mono out.

The package says 1/4" stereo to 1/8" mono. That's what I get for not reading the packaging. Thanks for the response.

Radio Shack might still carry these. Used to be my go-to.

Online: Monoprice, Parts Express, allelectronics.com, B&H Photo, Sweetwater all should have a variety of options at low cost, but what will be hard to stomach is that shipping will cost as much or more than a single adapter unless you pad the order with other items to meet the minimum order for free shipping, which varies. B&H and Sweetwater have free shipping for most items however, with no minimum.
