It is in his Chord Melody Book 1, available from his website (pay via PayPal and then you are sent a PDF). There has been some issues getting it since his passing, both his wife and daughter are trying to fill orders.

Please follow the Mike’s site and donate, if you do not hear back you can post the receipt as proof and I am sure some one from the community will be open to emailing the PDFs. I got his PDFs post donation from another member as well as his wife.
