Song Help Request Ukulele Ear Experts! HELP!

I'd like to learn this song (see URL below); there's no site that provides ukulele chords & it's also a cover. I'd like to learn it to play it for my youth group for praise & worship. The girl who did this cover is not responding to any of my messages/comments about the chords she's using.

I guess the girl tuned the uke down a half step.

The chords are F# D#m A#m B C# - if you tune your uke F# B D# G# you can play G Em Bm C D. If you have these chords you can figure out when she plays it and write down the chords.
