I'm a rank beginner as far as ukes are concerned, but as a guitar player, I know that manufacturers have their own, unique peg head designs.

How is it that Lanikai and Kohala both use the same basic design, are they related in some way?

Thanks for looking... Obviously nobody has the answer; sorry I asked.

There's only so much you can do with a peghead. That's just a traditional design. Some of the Lanikai seem to have a more rounded end than the Kohala.

Ah, when he said "peghead" I foolishly assumed he meant "peghead". So we're talking "manufacturer logo" are we ? Sorry I looked.

They look the same because they're the same brand--Kohala is the cheaper, (even further) overseas version, a la Fender/Squier. The packaging sometimes says "Kohala by Lanikai."

In the Guitar world Fender has, I believe, copyrighted their peghead design and sued some companies over it. So far no one has done that in the uke world. The little pointy crown is open for anyone to use.
