This is the best site i've found for staff paper of just about any kind. Just set it up the way you like and print it.

Try this site and use the one for mandolin. Staff lines and 4 lines for tab.

Not sure on that on Phil sorry...maybe you a program for that....I heard it discussed here before....can't remember who it was though...I am sure someone will chime in soon..

Yes. Guitar Pro. It is well worth the $50 if you make lots of tabs.

No, you can't put notes on it.
It looks like you can do it in Finale Notepad, a free, lite version of Finale notation software. http://www.finalemusic.com/products/finale-notepad/

It looks like it doesn't come set up with uke tab notation, though it appears that it can be set up with a little effort. http://forum.makemusic.com/default.aspx?f=5&m=203320

Yes it was me discussing it before. I would have thought somebody would have come up with it. Just a simple blank sheet where you can just go along and put the numbers on the lines. You can do it in Guitar Pro but it is a lot of faffing around just to do that.
