The order form is still up at http://www.ukulelemikelynch.com/order.htm . The site hasn't been updated, but the catalog was in Feb 2021: http://www.ukulelemikelynch.com/catalog.pdf . Oddly, if you want all six chord-melody books it's cheaper to order 3 + 3 rather than all 6 as once.
